$10 rebate on $30 Olay products purchase

Purchase $30 worth of Olay Facial Skin Care products between 4/28/13 - 6/30/13 in order to be eligible for $10 pre-paid card.  Print and fill out the rebate form here, attach your receipts and submit it before 7/15/13.  Limit one rebate per household.  The $30 minimum purchase is AFTER any coupons or discounts are applied, so your total out-of-pocket expenditure before taxes must be at least $30.
