Click here to download and print the rebate form. Mail the completed rebate form in with your proof of purchase UPC and the cash register receipt dated 5/31/11 through 7/31/11, and you will receive your full rebate within 8-10 weeks. If you've already purchased the Weight Loss System Starter kit outside of the rebate dates, you can apply for a rebate for a Refill Carton or Treat Refill with the same form. Form must be mailed no later than 8/31/11.
Seriously, even if your dog isn't overweight, because many dog breeds tend toward life-shortening obesity as they get older, it's a great idea to get your pet going on regular sized food portions and treats.
