Receive $5.00 by mail when you purchase 1 Lysol product and a flu shot. Receipts must be dated between 10/1/11 – 2/29/12. Mail the original, dated receipts showing purchase of 1 Lysol product and payment for the flu shot.  Click here to download and print the rebate form.

If your flu shot is not covered by health insurance and you pay for one out of pocket at CVS, CVS will give you a $5 CVS gift card. 

(If you don't have health insurance and live near one of these clinics, you can walk in and request a voucher for a free flu shot at any CVS location.)
