Purchase a bag of Science Diet Ideal Balance dog food before 12/31/11, and you can request a full rebate using this mail-in form, for up to $12.99 of your purchase price.  Must include original rebate form, UPC symbol from the product and original register receipt.

Joshua and I are going to be getting a yellow lab puppy in January, so I will definitely be picking up a bag of the Ideal Balance puppy food.  Free dog food?  Can't beat that with a stick.
