Purchase GE Energy-Smart LED light bulbs before 7/31/13 and you'll be eligible to mail in this rebate form and receive cash back on your purchase. Mail the product UPC symbols and dated cash register receipts in with the form and you will receive $5 back on your minimum $25 purchase or $10 on your minimum $40 purchase. Limit one rebate per household/address.
